Form Maker Pro 4.5 is available in a PHP + MySQL version to run on any Windows- and Unix- compatible platforms (web-server) and meets the standard requirements for PHP scripts. This means that usually there are no additional installations needed for the final server.  
Form Maker Pro requires PHP 4.1.0 or higher and MySQL 4.0 to be installed on the system.  
All packages are commonly available on the hosting server.
